Both tenants serve /enrolment/new and byte-diff to the HIG form apart from logo, tenant id and a style block (checked 10 Sep 2026).
New Patient Questionnaire
Not confirmedMeasurements, the medical history grid, medications and allergies. Opens in place from the chooser on the incumbent; here it also has its own URL.
The vendor form is the same build, so the chooser probably follows enrolment here too — but no MHT capture shows it. P-HIG-033 is the MHT recording; playback would settle this.
Self Check-in
East Care onlyThe waiting-room funnel: find yourself, confirm your details, say why you are here. Three recordings are three paths through one set of screens.
